Wraparound Program

Wraparound is a planning process for adoptive families that follows a series of steps to help children and their families realize their hopes and dreams. The Wraparound process also helps make sure children and youth grow up in their homes and communities. It is a team-based process that helps groups of people involved in your family’s life work together toward a common goal. With help from a Wraparound facilitator, people from the family’s life work together, coordinate their activities, and blend their perspectives of the family’s situation.

4 Phases of Wraparound

Engagement

Along with the Parent Partner, you will explore your family's strengths, needs and goals. Together you will discuss what to expect from the wraparound process. The Parent Partner engages other team members and prepares the group for the first wraparound team meeting.

Initial Plan Development

This is your first wraparound team meeting. Your family and team will meet to discuss your family's strengths, needs, vision, strategies and action steps to create your wraparound plan.

Plan Implementation

Your family and team meet regularly and work together to implement the plan. You will discuss accomplishments, and assess if the plan is working to achieve your family's goals.

Transition

As the term nears its goals, preparations are made for your family to transition out of formal wraparound. Options that will help support the family to succeed outside of the formal wraparound structure are brainstormed and a transition plan is developed.

Our Wraparound team is truly dedicated to our families to make every effort to help children and youth grow up in their homes and communities, without the need of a higher level of care outside the home. Our team is here to improve your well-being and ensure a safe, stable, and permanent family environment.

FAQ's

Who Qualifies?

The primary qualifying criteria are that the youth has been adopted out of foster care and is experiencing mental health or behavioral challenges. Each county of origin makes the final decision regarding your family’s Wraparound qualifications.

What is the Cost?

There is no cost to your family to receive Wraparound. Funding comes from state Adoptions Assistance, and through billing insurance. Your family continues to receive your monthly Adoption Assistance Payment and it does not interfere with existing family services.

How to get started:

Contact Kindred Hearts to request an intake session to see if our services are a good fit for your family. Final approval for funding will come from a County Adoptions Worker, though we work closely with your family and your worker on the approval process.
